Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

This position supports a busy US‑based cardiology team by acting as a patient navigator. Working remotely, you will manage communication, scheduling, and care coordination for a high‑volume caseload of over 150 patients, ensuring they stay connected to their treatment plans.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows within EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
• Assist patients navigating cardiology pathways, including heart failure management protocols.
• Track testing schedules, appointments, and remote monitoring workflows.
• Coordinate with clinical providers to maintain continuity of care and escalate concerns when needed.
• Maintain accurate clinical documentation and ensure HIPAA‑compliant records.
• Support billing documentation and insurance coordination as required.

Required profile

• Clinical credential such as CNA, MA, RN, or equivalent.
• Minimum 3 years experience in healthcare, telehealth, or medical office settings.
• Proven experience in care coordination or management, preferably in a remote or telephonic environment.
• Ability to handle a high‑volume caseload with strong organizational skills.
• Excellent phone etiquette and patient‑first attitude.

Required skills

• Experience using remote patient monitoring platforms.
• Knowledge of HIPAA compliance standards.
